PHP how to add some username and password and button

hello anyone, can you please help me how to put some textbox and button for the login

 

just a newbee here.
i convert the index.html and style.css into text for just to upload in here.

 

Hi Paul! As far as I know creating a login system will take a little PHP/MySQL knowledge, so it's not as simple as just uploading your HTML and CSS files here, sorry! Do you require this on just the index/home page? Or would you require the option throughout the website?
