Nice template, but I don't think that the whole box thing acctually works for this template.... but the images look real good

i think u creat that page is nice but that page cliarity is not good like some mistek in image so plz chek it.

i was download your template, looking very nice but in this template both hand of lady is different and not sharp border image.

You need to use a script to capture the data in the form. PHP script is very popular. The first line for the form can be something like this:

Note that it is better to use post method instead of get.
